If you are claiming a free-issue subscription, please specify the reason, choosing one from the list below. You must have previously have agreed this with BCA or BCRA and obtained an 'authorisation', which you should enter in the box. Any unsolicited forms that we receive, claiming a free subscription, will not be actioned and they will not be queried with the applicant; they will simply be deleted unread!
